Why Perimenopause Makes Your Shoulders and Hips Ache

If you're in your 40s or 50s and suddenly dealing with a shoulder that won't lift overhead, or a hip that aches for no clear reason, you're not imagining it, and you're not alone. Many women describe this exact scenario during perimenopause: seemingly random joint pain, stiffness that wasn't there before, and injuries that “come out of nowhere”.

There may be a real physiological explanation for this that can be traced back to a hormone most people associate with periods and hot flashes: estrogen.

Estrogen Isn't Just a Reproductive Hormone — It's a Fascia Hormone

Fascia is the web of connective tissue that penetrates every muscle, bone, nerve, and organ in your body. It's what gives your tissues their glide, their spring, and their ability to move smoothly against each other.

What many people don't realize is that fascia is a hormonally responsive tissue. The cells that build and maintain fascia (fibroblasts) actually carry estrogen receptors. That means fascia is, quite literally, listening to your hormones and changing its structure in response.

When estrogen is at healthy levels, it helps fascial cells produce more elastin and type III collagen — the stretchy, resilient building blocks that keep tissue supple. It also supports hyaluronic acid, a lubricant within the slippery "ground substance" that lets layers of fascia glide over one another instead of sticking.

What Happens When Estrogen Drops

As estrogen declines during perimenopause, that whole system shifts. Fascial tissue tends to produce more of the stiffer type I collagen, while the stretchy elastin breaks down faster than it's replaced, all while the lubricating fluid within the system dries out. The result is fascia that's less hydrated, less elastic, and more prone to forming adhesions, restrictions, pain and other symptoms.

While this phenomenon isn't isolated to one area, it tends to show up first in the places already under the most mechanical stress: the shoulder capsule, the hip, the plantar fascia of the foot. Hence why frozen shoulder and unexplained hip pain are so common in perimenopausal and menopausal women. Research has also linked declining estrogen to reduced tendon and ligament elasticity more broadly, along with the same collagen changes that make skin thinner and less supple during this life stage.

Why This Often Gets Missed

Because these symptoms show up gradually and in seemingly unrelated places — a stiff shoulder here, an achy hip there, a cranky heel — they're easy to write off as separate, unrelated problems, or just "getting older." But when you understand the fascial web is one continuous system throughout your body, it makes sense that a hormonal shift affecting connective tissue everywhere could easily produce pain and restriction in multiple spots at once.

It also explains why these issues often don't respond well to treatments aimed at a single joint or muscle. If the underlying tissue quality has changed throughout the body, addressing one isolated area rarely gets to the root of it.

How Myofascial Release Can Help
